Although the evidence base for Lifestyle Medicine is strong, and the national need for Lifestyle Medicine implementation is enormous, financially viable models for delivery of Lifestyle Medicine by private health care providers remain scarce.  We discuss a new Lifestyle Medicine practice model that is based on a Direct Primary Care reimbursement approach.  Specific components of the clinical model will be covered, including:  incorporation of health risk assessments, other focused LM diagnostic tools, LM customized electronic medical records, personalized health goals, behavioral counseling, group visits, ITLCs, and other LM clinical resources.  Details of the financial model will also be reviewed, including:  determining panel size, calculating rates, creating schedules, staffing patterns, marketing, locating financial partnerships, billing resources and other components of successfully operating the business of LM.  Although we are still in the early stages of implementing this practice, we believe that what we are learning has value as we look for ways to bring the Lifestyle Medicine choice to the average patient and health care provider.
